New Service Authorization Form February 5, 2010 In our continuing efforts to better serve our clients and their employees, we have recently updated our Service Authorization Form. We strongly encourage our clients to send a completed copy of this form with their worker for any clinic visit. This will help eliminate any confusion on which services are to be provided and subsequently reduce their wait time. You can find the new Service Auth on our forms page.
